Title IX Reporting Form

Use this form to report incidents of sexual harassment, sexual assault, domestic violence, dating violence, stalking, or sexual exploitation. State law requires faculty and staff to report any sexual assaults they are made aware of, regardless of the location.

You may make an anonymous report, which means you inform someone that you have experienced sexual violence without identifying yourself or providing sufficient information to determine your identity. Please note, however, that TRCS may be limited in its ability to investigate or respond to the incident if it does not have sufficient information to follow up on. Every effort will be made to protect confidentiality, when possible, particularly where a safety concern exists.

Reports are typically reviewed within one (1) business day. Once the report is received, a Title IX Coordinator initiates a preliminary investigation. Individuals identified in the report might be contacted to provide further information.

If you have any questions regarding filing an incident report, please contact the Title IX Coordinator at 269-279-1100 or titleixcoordinator@trschools.org.

DISCLOSURE: The District will undertake an objective evaluation of all relevant evidence – including both inculpatory and exculpatory evidence – and provide that credibility determinations will not be based on a person’s status as a Complainant, Respondent, or witness. It is a violation of District Policy for a Complainant(s), Respondent(s), and/or witness(es) to knowingly make false statements or knowingly submit false information during the grievance process, including intentionally making a false report of sexual harassment or submitting a false complaint. The Respondent is presumed not responsible for the alleged conduct until a determination regarding responsibility is made at the conclusion of the grievance process. The District prohibits retaliation against any person who reports or files a sexual harassment complaint or who cooperates in the investigation of such a complaint. Retaliation can result in disciplinary action up to and including discharge or expulsion. Retaliation allegations should be reported to the Title IX Coordinator. A person alleging discrimination by the District on the basis of sex may file a complaint through the District’s grievance procedure. A complaint may also be filed at any time with the Office for Civil Rights (OCR), U.S. Department of Education, 1350 Euclid Avenue, Suite 325, Cleveland, OH 44115. Filing a complaint with the District is not a prerequisite to filing with OCR. For additional information about the District’s grievance procedure, please contact the Title IX Coordinator.
ACKNOWLEDGEMENT: By electronically signing this form, I acknowledge that I am filing a Title IX complaint and am requesting to initiate the grievance procedure under the District's Title IX Policy.
